Job Description

Our teams work to address each individual on a holistic level so they feel heard, respected, and safe in our care. We offer hands-on help to address chronic homelessness, mental health, substance abuse, disability, and systematic poverty by offering innovative solutions and proactive services to the most vulnerable in Schenectady County.

The Transporter reports to the Day Program Supervisor and supports services to address Health Related Social Needs (HRSN) under the New York 1115 Waiver Amendment which include a broad range of housing, case management and nutritional support.

The Transporter will respond to referrals from the Unite Us platform providing transportation using the Agency’s bus or van to transport clients for non-emergency services, including trips to the grocery store, delivering food items from food pantries, addressing shelter needs, and meeting complex needs such as transportation to health care. 

Primary Responsibilities include:
  • Delivery of clinically appropriate home-delivered meals
  • Provide transportation for individuals and groups to medical appointments, grocery stores, farm stands, food pantries, and housing-related services.
  • Complete all required documentation promptly, including daily entry of participant activities into the electronic database.
  • Support the program in meeting established monthly and annual outcomes.
Qualifications
  • Associate’s degree in human services or equivalent work experience in a related field.
  • 3-7 years of experience providing transportation services to individuals and groups safely and effectively is required - highly preferred experience transporting those who are experiencing homelessness, economic hardship, mental health challenges, or with substance abuse history. 
  • Must have a valid NYS driver’s license with a safe driving history for at least 1 year with no DWI or DWAI within the last 3 years.
  • Must be able to successfully complete the Agency’s road test and qualify for coverage under the Agency’s vehicle insurance.
  • Must have access to reliable transportation to and from work
Skills and Abilities
  • Strong written and verbal communication skills, with the ability to engage with individuals in both routine and high-stress situations related to medical conditions and social determinants of health.
  • Proficiency using computer systems, including data entry, analysis, and interpreting automated output.
  • Skilled in conflict resolution and managing confrontation to achieve mutually satisfactory outcomes.
  • Demonstrated resourcefulness, ability to work independently and take initiative.
  • Familiarity with local agencies and programs serving homeless and low-income populations.
  • Able to meet deadlines and manage workload in a fast-paced environment
  • Professional demeanor and ability to positively represent the agency at all times.
  • Consistent availability during standard business hours, with flexibility to work beyond assigned schedule as needed.
Schedule : Monday – Friday
Job Type : Full-time, Non-Exempt (30 hrs/ wk)
Pay Rate : $25.65 / hr
